How can I take a form that is shared with me and save it?

I took a form from the shared list and revised it to make it my own.  However, when I saved it, it was saved under the shared list and not under my list.  How can I move the form to my own forms list?

The only way to make it belong to your account is by having the owner, the account that currently host the form, to contact us and request  to transfer the form over your account.

So, in a new thread opened by the owner, he needs to specify which form will be transferred and provide the username of the account were the form will be transferred to.
