I'm looking to have an online form (that can be viewed on mobile) that once submitted it will send the response with the questions and answers to the recipient as an editable form. So that if a date or time had to be edited they can edit it right on the form as opposed to going back and submitting a new one. Ideally it would be exported as an excel file or a word template.
