Form loses Design Style when Submitted or Opened

    To my understanding you are having an issue with your form losing its style.

    However, after trying to check your provided form link it appears you've deleted the form.

    Do you still need help with this problem? Please let us know.

    Hi Kade,
    Thanks for getting back to me. Apologies for the incorrect URL, I had
    deleted this test form. The correct URL is:-

    Please see the screen shot of the form when I open it after submission.

    Hi Tom,

     

    I did a test submission in your form but I was not able to replicate the issue you are reporting. Also, I was not able to see the screenshot you mentioned. Can you try to attach/upload the image in this thread? You can follow the instructions provided in this link to upload the image on this thread: How to add screenshots images to questions in support forum.

    Hi Christian,
    Thank you for your email. I have attached a PDF version of the dummy
    submitted form generated from within my Jotform site. As you can see this
    has lost some of the information (Page Header) and is not formatted exactly
    as the original in my JotForm forms.

    Please revisit your thread here in the forum and reattach it via our Image Insertion Tool or via Our Image Repository so that we can better understand the issue that you are currently trying to show us more about.

    Apologies, I meant to also mention that alternatively you could just email it to us at support@jotform.com with a reference link to this thread so that we know it is you reporting it there and we can take a look at it that way too if that is easier for you.
