The forms were disabled because the account was suspended by our anti-Phishing system. This is most probably because of the labels in your form "Facebook" and "Hack".

It would be best to avoid using the word "Hack" in your form.

Also, the form is on a non-English language, it would be best to convert it to English first then follow this guide on how to make in multilingual: http://www.jotform.com/help/298-How-to-make-your-forms-multilingual. Why is this? We got a tool called Phishing Detector which allows us to automatically suspend thousands of forms created everyday for phishing activities such as stealing credit card information, social security numbers, login credentials, child pornography, etc. However this tool is not updated to scan Arabic,French and other similar languages, therefore most of the forms made in these languages are automatically suspended.
