Form disabled due to suspected phishing activity.

    I'm sorry, I'm not sure I understand your concern completely. It seems that your post is incomplete. Are you asking how you can disable your forms? or you have a disabled form that shouldn't be. Can you please help us by giving us more details of your concern so we can assist you further? We will wait for your response. Thank you.

    Thank you. Yes, the form is becoming automatically disabled  :( I created forms on two separate accounts very recently... I made sure they're enabled under "preferences" and under "my forms".. both show they're enabled but it very quickly says "form not found - form disabled" :( The website I use to embed is www.map-fitness.com (wordpress) at the bottom of the site, but I know its not the site doing it, there's got to be an issue with Jotform... eek!

    I'm sorry. I made some further research on your account and see what you mean. 

    It seems that your forms were automatically suspended because it was suspected of phishing activity by our anti-phishing system. I have verified that the forms were good and have whitelisted it.

    Our system has marked it because of the word "pass" in your header, which may be related to the word "password" that is not allowed to used in forms, especially when combined with a "username".

    Your form is now enabled and you may continue to use it again. We are sorry for any inconvenience this have caused you.
