How can Smartponie user submit form and have it updated to our App?

We are building an App with Apps Bar and we would like the Smartphone user to be able to use your jotforms embedded on our website to load in key information, then save it to their Smartphone.
Right now any information loaded in is sent to my email address, I want it to upload to the Smartphone users cell phone. I have seen other companies do this and they do not link it to email but link it to the Smartphone users cell number somehow.
We want to make our App so it can be edited by the end user. It is very similar to the Pig Geico App commercial but we are trying to build a digital Pet Health Card where the Pet owner can enter, update, and save the information so they have it available at all times.

We do provide API and Libraries which can be used for custom integrations. Please note that this is a quite advanced topic. I believe that you will need a developer to implement such integration.
