    Missing a form

    My main form, Services for the Home has disappeared from My Forms, although the summary and grid reports remain accessible with these two links on my own website:

    When I try to access the input form, the "report card," I get a pop-up saying it was deleted by the owner, which is not the case.  I never deleted it.

    The input form that is missing had this url:

    Is there anything I can do to restore the input form?

    Not sure how it got there but the form in question was indeed in your Trash folder. I have restored it for you but should you ever need to restore another form, please see this guide.
