    I am not completely understanding how to make sure that I get a notification of submission and that the person filling out the form gets the thank you.  Also, I need to add another person to receive the submissions.  How do I do that?  Thanks.  If you click on the url below and click on Enroll on the home page you will see the form.

    Hi,

     

    I see that you have already setup a Notifier and Auto Responder. Can you try replacing the Sender email to noreply@jotform.com?

    Also, I noticed that your form is collecting Credit Card information which violates Jotform's terms. I have deleted it the said fields. Please don't add those fields again to avoid your account from being suspended.

    If you need to add a payment functionality in your form, please use the Payment fields available in JotForm such as Paypal, Stripe, etc. You can check this article for more information: Order Form Basics.
