Export of form

    We are looking into moving to using the service for our medical release forms that we use here. What I am looking for, is there anyway to take the form options that you have, but to input those fields onto a PDF or JPEG image that I already have. I want the text boxes to fill the lines that I have blank and then to export to us as the actual form that we already have. Is this possible with your site?

    Hi,

    I am not quite sure what you are looking for.  If you are trying to have someone fill out a form on JotForm, then have those fields populate a JPG or external PDF you have already created, that is not currently possible. 

    If you are simply trying to print the full form, you can add a print button to the form:

    If you would like to include the full headers and text of your form in the PDF output, here is how to do so:
