I can't login to my JotForm account anymore. It appears that my account "has been deleted" and I don't know what to do, because I did not delete it.

Please note Jose that your account was deleted as per your request: http://www.jotform.com/answers/292923
As such you would need to create a new account, but for our own security purposes the old account username is under lock so you will need to use a different username, possibly the email as well while creating the account (but you can change it later on to anything else).
