    Can I include a unique URL

    Basically If I wanted to create a comments form which used the same form across 1000 products but in order to understand which product they are leaving a comment for can the form submission includes it URL or i.e URL Page name

    You should probably add a hidden field to your form. Then you can pre-populate it from the URL.

    Example: Let's say you added a "Page Name" hidden field to your form. Here is what your URL would look like:



    Note that "PageName" is usually whatever you entered on the question minus the special characters like spaces. But if you are not sure you can find it out from the email wizard.

    i think i understand but do you know what html string i need to add with over 2000 products i would need the form to be placed within the product template but to send me the url it came from so i could determine what product they are looking at as each product is listed in the url

    Well, then you will need to populate the product name into the URL dynamically. How do you show the product names on the page. If you use php, you probably show it like this:

    <?=$product['name'] ?>

    If you are doing this, then your link would look like this:

    target="_blank" href="http://www.jotform.com/form/123456789?PageName=<?=$product['name'] ?>"
