i have a form that is saying form not found it has been disabled what to do

I checked you account and it appears is was suspended by our phishing detector because some forms were flagged as  collecting sensitive user information, namely payment details

Kindly remove the fields for credit card payment collection in the following forms

1. Agent Payment Form (remove the portion where you specify the card type)

2. Make a Payment Form (Has fields collecting credit card information)

Once you do this you account will be reactivated.
