option checklist

I'm trying to build a simple order form for repeat customers.

I like to have an extra field to pick quantities right next to my option checklist, where I will list my products.

Can you tell me how to do it?

Could you please elaborate a bit more on how your form will work? I just reviewed you ORDER FORM, and would like to know if you are making a form to sell products? If this is the case I recommend using one of you payment gateways. This will help you create all the billing fields require to sell products.

If this is not the case, the only way to create a quantity dropdown next to the list, is to create carious checkboxes and add a dropdown field next to each checkbox.
