How can I get my customers to open a customer login only?

Is their any type of form that new customers can fill in a user name and pass word to become members, and only the customers that can fill in my book on line form.

Unfortunately, these kind of forms are not allowed here in JotForm. You cannot create a login forms because it does not membership feature.

Forms that are asking sensitive information like passwords and credit card information are automatically being suspended by our phishing detector tool because it violates our terms of use.

Though you can create a registration or sign up forms as long as there is no password field.
