probleme formulaire a disparu ! deleted form please help

doesn't work today. i've created it yesterday and some people could fill the form but not today !

and the report https://form.jotform.com/53081734955966 have lost teh submissions :(

how can i re collect information. little bit important it is for a gift for a collegue of the company

I have looked at your account activity logs, Sebastian, and it looks like its fields have been accidentally removed.

I can see in that form's revision history that the fields have been removed 2 hours ago. Don't worry, you can simply use our Form Revision History to restore your form to its revision from before the fields were removed. When restoring the fields through the Revision history, it will also restore data for these fields in your submissions page.
Please let us know how it goes, and if reverting the form to its prior revision resolves the issue you are experiencing.
