I used my android smartphone and checked your webpage where the form is embedded. The form is showing properly however, it doesn't look good because of the form tabs and header image.
Please delete the "header image". You need to use the "Image" tool and upload that image.

I'm sorry, perhaps my question was unclear. This was related to my other 2 issues. I removed the image, but the problem was that when the first page of the form was filled out and the "Next" button selected, an alert kept saying "There was an error and the page was reloaded." This occurred on 3 different phones.

I've checked your account and I've found only three forms, I made some tests but I'm not able to replicate your issue, please , could you try again ?
If the issue is still there, could you provide us the link to your form ?
