Not all responses to questions are coming through in the email notification

    I run a summer music camp, and I received a complaint from a student that not all of his responses in the Scholarship Application template came through.  I attempted to fill out the form myself, and found the same problem.  In the screenshot attached, you'll see that despite having filled out all the boxes in the form (some of which are required), that not all of the answers were received, and many blank boxes remain.

    I tested and could recreate the same situation. Please create a new email notification. From my tests this will solve the situation: 

    Let us know if the problem persists.
