I am having trouble logging in this morning- I am not sure why.
I do have two emails that come in to the same inbox (info@artsandservices and bestrada@artsandservices) but I cannot seem to get "reset instructions" to be sent to either one.
Any help is greatly appreciated.

I've searched your account using the form-id and it seem that you didn't create an account. You are now using JotForm as guest. That's why you didn't receive the reset instructions via email. You just need to create a free account using the email address above(bestrada@). Your form will be automatically transferred to your new account.
