How to display textbox for every checkbox options?

    here's what I need. I want to have a form that allows someone to click a box, or circle (it doesn't matter) and then be able to put in some text next to it. 

    For example, I will be sending a form to a high school and I want to know if their sport teams have facebook, twitter, youtube social channels. I want there to be a category for Football with all the social channels listed below and they would check the box or circle if they have a social for that sport and then in the text box next to it they'd put in the address or info for that channel. Then the same for Volleyball, water polo, etc.

     

    I hope that makes sense. is there a specific widget for that???

    Hello Casey,

    Unfortunately we do not have such widgets but with a little workaround, it is possible to achieve your requirement. You can add text box for every social media option checkbox. You can then add "Conditions" to show hide the appropriate textbox based on the textbox selection.
