What is your JotForm account username or the email address used in your account profile? I am unable to find any details with your email address email@example.com.
Alternatively, you can also let us know any of your form ID or the web page URL where you have embedded your form and we will be able to trace your account.

It was indeed the form that was asking for credit card details that resulted in the suspension. We do not allow for the collection of such details directly in the form. If you need payment authorization, I would recommend doing so via one of our payment processors.
I removed the credit card fields from the form and reactivated your account. You should be able to access your forms once again.
