This is the format of the link:https://form.jotform.com/joepolonsky/fundraiser
How can I change "joepolonsky" to another name? for example the name of my company which is utcice.

As you can see, the part of URL with "joepolonsky" was actually your account/username.
If you want to change this on the custom URL name of the Form, you will have to change your username.
Only us members of the support team can change account username. If you request to change your username, we can do it. But please take note that changing your usernam will also reflect to all your forms, not just one.
