Your account/username   was already Suspended by the Phishing filter in place because of the phishing form you have created.

You form violates the JotForm Terrms of Use.

Please do not create such form to avoid getting permanently ban from using JotForm services.

Just a comment!! :-)   This is obviously created with bad intent so I don't understand why you wouldn't immediately ban such a user as there can be no good purpose for a form such as this?

Having said that - keep up the good work at JotForm!!

We appreciate your feedback  msimpy . Thank you for taking the time also.

But yes, the form and the account were immediately detected and got disabled by the Phishing filter on this case.

Just to mention, we do have form reviewers that also manually review forms and accounts.

We do take into consideration that a user might have unknowingly copied, used, or assigned with form that are considered forbidden or illegal.

For such case, a user do deserve understanding and can be guided to correct the mistake. Most of the time, they do end up good users as well.

    Cheers
