I embedded this form onto the backend of my website (Wordpress). The form translates fine, but when i hit submit, it takes me to a blank webpage and the responses don't come thorough. Any idea why? Below is the code I pasted.

It seems like you are using the form's full source code. If you wish to embed your form on a Wordpress website, you can follow this guide: http://www.jotform.com/help/324-A-Guide-To-Forms-For-WordPress-JotForm-Users. See if any of the methods mentioned there will work, I personally recommend using the iFrame embed code. We'll wait for your response.
That doesn't seem to work.
Can you please let us know Sarah which code did you try and it is not working for you?
Also did you try other alternative methods covered by that guide?
Based on the source code that you have added above (which I have now removed since it takes a lot of space) I can see that this is the form that was embedded on it: http://www.jotform.us/form/53086482206152, but it would be best if you could give us the link to your page once you embed the form so that we can check it out directly and see what might be happening as well.
