Upon further investigation, it seems like it is more related to the custom sender email address email@example.com that you are using.
You can check your email history by following this guide: http://www.jotform.com/help/293-How-to-view-all-your-form-Email-History. Based from my observation, it seems like the failed emails happened when you are using the email address firstname.lastname@example.org as the sender.
You might need to contact your email service provider to whitelist our JotMail IP addresses or domain name servers mentioned here: http://www.jotform.com/help/145-Whitelisting-JotMails-IP-Addresses. You can also use this guide as a reference if you are still unable to receive the notifications: http://www.jotform.com/help/208-How-to-setup-email-alerts-to-prevent-email-bouncing-related-issues
