Before I cancel my subscription to Jot Forms, I wanted to present my concerns about this product one last time.
On several two different computers and multiple browsers (Internet Explorer, Google Chrome, and Microsoft Edge), Jot form becomes unavailable and times out. Not to mention the reoccurring issue of the latest version of my form disappearing. I have cleared my cache and still have these same issues, even when on a different computer.
In addition,when viewing one of my recently completed forms, more often than not- I am unable to navigate through the different tabs of the form. When I click on the tabs, nothing happens. This has occurred more often than I am comfortable with. Also- there is an error message that shows up at the bottom of my form (see screenshot)
My forms will essentially be used my dozens of users at one time and I cannot have these issues occurring while they are attempting to input their data.
Can you explain to me why this is happening and what will be done to resolve it?

May I inquire if your jotform http://www.jotform.us/form/52868565777174 was used or embedded on a website? If it is, can you please share also to us the URL of your website where we can also test the form used.
Currently, I could reproduce the issue of the disappearing form or unavailable issue. I checked and test using the form URL on different browser, but I was able to see the loaded form just fine.
Can you also take a screenshot image of the form not available on your browser when you encounter the problem? Was there any error message seen on the browser when the for did not load? If you can share an image to us, it will help us understand more where the problem might be.
Please let us know if this issue persist.

Yes, that's the correct form. It was never embedded into a website. We recently finished building it. Here's an example of the error message that was received:

I loaded the form http://www.jotform.us/form/52868565777174 and checked the browser console, and I see red script error as well.
From what I can see, the script error originates from the form source. So this can only be fixed on our end. It seems also that the error was causing failure to the forms loading properly on some browser, similar to your browser.
I have submitted a ticket to our higher team support to look into this. Hopefully it can be fixed by them the soonest possible.
