We have had two forms in the last week where the form fields have disappeared and the form was blank. When looking at the revision history, we see a date and time where all the fields are marked as "removed" yet no one did it. We are able to revert to a previous date when all the fields were present, however we have no guarantee that the form was totally up to date at that time.
Form name - CDC - /web/cdc/request-info - CDC Request Info Form
INST - /web/registrar/reenrollment - Registrar Re-enrollment Form
Please help. We do not want it to happen again. Thanks,

Please accept our sincere apologies for the inconveniences caused.
We had a database switch about two weeks ago which is probably a reason of the removed fields (blank forms). We do not expect this to happen again.
It is also recommended to check Account Logins history to make sure that there are no any unfamiliar IPs.
