My form has changed and I didn't

The whole interface AND the look of my form has changed. Someone should have tested this before changing stuff AND NOT telling us.....

You can check the history of your form, you will see if there is a change and you will be able to restore it to a previous version :

Please read the instructions provided at this link. There is nothing under the "more" pull down about form history......

The guide my colleague posted should indeed be correct.  In the My Forms section of your account:

Select a form, then click on the "More" tab.  Revisions history should be the fourth option down:
