5) Right - I'm not sure whether I want collapsible sections of a form or whether I want tabs - I've currently got a tab widget on this form but I don't think I've got it in the right place to work.
Ideally I want to have the first page or tab to request the primary details (as I currently have in the form) and then two dedicated sections/tabs for a legal participation agreement and a health history form. Each of these two sections will have a whole series of conditional logic, checkboxes and so on that 'guide' the user's navigation of the form.
Here is what I want to achieve - I want to have the full conditional logic functionality of Jot Form for these sections, but then I want it to be able to send a fully formatted PDF of the agreement and health history by email to the user upon form submission. I want the PDF to reflect all data entry, including the electronic signature I will capture from them.

We will need to have a final design of your form so that we can advise on how to set up the conditions for the mentioned sections.

Regarding sending a copy of the PDF to the form submitter, it is possible to do so via the autoresponder. However it is not possible to edit the layout of the PDF as it is auto-generated during submissions.
