You can check this form I have created:

On the Code field, enter 1234 to show the rest of the form and the submit button.

Here's how you can re-create that form.

Feel free also  to clone that form using this guide: How-to-Clone-an-Existing-Form-from-a-URL

First, create the original content of your form.

Then, add another field on top of the form for the Code field.

Note: Field name such as "Password" is not allowed to be used within forms here in Jotform. This will make the form marked for suspected phishing activity. Please refer our Terms of use for more information. To avoid automatic suspension of your account, we advise you to rename your "Password" field to "Pin, Code or Key etc...

You will also need to have a Form Collapse to show/hide the content of the form as a whole(not per field).

Then, set the Form Collapse as hidden with status as opened so that it will not show on the form and it is always open to show the form.

Once you've done that, it's time for you to set up the condition that will show the form if the code is correct. The condition will state that "If Code is equal to 1234, show "Show Form (Form Collapse)".

    And that's it.

I hope this helps. Let us know if you need any further assistance with any of the instruction above. Thank you.
