since last month my clients have been saying they are not able to successfully complete the form.
I get the the submission but they experience an error and submit multiple times. (see image)
this is costing me customers and their user experience is not positive.

Your form is working just fine. It is your thank you page that is down and this is where your users are redirected to after a successful submission that's why they are seeing the error page.
So I check your thank you URL and it is also down on my end.
However, even though your thank you page is down, the form should be processed successfully since thank you URLs are processed after a successful submission.
