I want to create a form that can be used for a 30+ event catalogue. I will need to give the info on the forms to my desiugner as a word doc for them to design the catalogue.

Could you please elaborate a bit more on what you are looking for? If I understand you correctly, you would like to hide the question when the user has answered the question. If I am wrong, feel free to correct me.
I have created a small form using a HTML TEXT field and a text box right underneath. Once you have answer the question, the question will disappear. This is done with the show/hide condition

You may clone the form if you like.
Please let us know if this helps.

Victor, this is helpful. Let me try to be more speciific.
I want to create a form with 5-10 answer fields. Can I download the answers for each form as a word document WITHOUT the questions appearing?
In the example you produced, I see that QUESTION 1 disappear when I answer it but would the ANSWER COMMAND still be present when I save the form or would just the actual answers that I entered be saved. I just want the entered information, not any of the questions/prompts.
Thanks
Joey

Hello Joey,
In my colleague's test form, yes the label "Answer" will still be displayed in the submission email or when you download the submissions in PDF or excel (as column header). If I understand the question correctly, you want the label also not to be displayed. Is that correct?
In that case you need to just not add any label in your questions. Please check the screenshot below:
Please take a look at the following cloned form of my colleague and see if this looks acceptable: https://form.jotform.com/53203473539960?
Feel free to clone this form for a closer look. The following guide should help you in form cloning: http://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL
Do get back to us if you meat to ask something else.
Thank you!
