I believe that you are referring to your JotForm http://www.jotform.us/form/31614451103137. I've checked your JotForm and I notice that the sender email address is left blank. I've changed the sender email address to use JotForm default email address. Also, I've tried sending a test submission and see that the notification sent to your email address without any issue from our end.
