Submission email which is being received is of original form template.

I have designed this form and have copied the link to my website

If you click on https://form.jotformeu.com/angellahornerhotmailcom/co3-student-exhibition-online-appli it will take you to the correct form to fill out i.e. 

https://form.jotformeu.com/angellahornerhotmailcom/co3-student-exhibition-online-appli

I then tested it by completing the form and sending it. When it arrived in my email It was not my form but a different form (looks like the original template I used to design my form from )so I must have done something wrong. 

Can you help please?
