We manage a holiday time charity and receive inquiries through our website. In advance of the holidays, we just tested the jot form (submitted a request to myself) and it did not work. Need help fixing that.Page URL:

I've checked your website link, and it seems like the form fails to load or does not work properly due to a script conflict or an error in your website. You can check at the screenshot below that the website is having errors at the backend. In my case, it appeared earlier when I first viewed your website, but on the second try, the form is not loading anymore.
To avoid script conflict, could you please re-embed your form in your website using the iFrame embed code? Here's a guide to follow: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code. Make sure to remove the old embed code before adding the new one.
