The "Please contact me by" field below on my form is not working. I have tested it many times and when I receive the email notification, the field is blank.

I'm a bit confused. Initially, I didn't see any form on the page to which you provided a link but quickly realized that the form was actually located on your Contact page. Then I didn't see the field to which you referred. So, figuring it may have been conditionally hidden, I went into your account to see if I could see the field there. Well, it was indeed hidden:
However, there are no conditions in the form to show it. So, how exactly is the user able to see and complete the field? Is it being pre-populated in some fashion? Please clarify.
