I checked your form http://www.jotform.co/form/41670721183855 and I found that it belongs to a Guest account only.
I recommend transferring the form first to your official registered account/username Kn9clipz
We will be able to properly fix the issue once the form is on your account.
Transferring the form will include also the existing submission data. So this will not be a hindrance.
Please let us know when we can proceed with the transfer.

Did I need an account? I only get around 5 bookings a week at most! Because this was set up by someone else, I don't know how to sort this problem . Where do I go to? How do I find my password?

I have moved mentioned form to your account Kn9clipz. Now you can see the form on your account.
Your users are unable to submit your form because Mobile Responsive widget is set as required field inside of your form. If you turn off Required option your users will be able to submit your form.

I have logged in to your account and turn off Required option for Mobile Responsive widget.
After that I have tested your form and I was able to submit it without "incomplete values" error message. I got redirected to thank you page.
Please test your form to see if you are able to submit it now.
