Question about text not showing up on submitted forms

Why are the actual terms and conditions (the written out text block) not showing up on the printed version of the submitted form we created? The box that says "I agree to the terms and conditions" is showing up, but I believe we need the full text of those T&Cs to show up on the printed form that we file, as well. Any help you can offer would be appreciated. I don't see how I can manipulate this on the custom form. 

I presume when you say "printed version", you are referring to the PDF submission? Is that correct?
