I have a form that conditions are being bypassed. please help

My customer acquisition form is malfunctioning.

I have had two instances of non-qualified customers ( entering a wrong zipcode) to get access to scheduling

Please review and let me know how to fix it.

i have tested it on multiple browsers and mobile and no problem but somehow people are able to click submit when the submit button should be hidden.

The last two submissions are case in point- how were they able to defeat my qualification system- is a problem with the code or the form or what could it be?
