4-Column Alignment Issue

    Hi there,

    I'm trying to get this content to display in 4 columns, but there's an issue with the alignment of the checkboxes and field entires on the far right column. Could you please let me know what I'm doing wrong? Thanks :)

    I checked your form and it looks fine to me.
