I've checked your form, which is http://www.jotformeu.com/form/53221590849358, I am not able to replicate the issue that you are having.
I've tested your form on IPhone 5s. It works fine on my side.

Hello, thanks for your reply. Yes, I did embed the form to my website, here is the url www.successtodaygroup.com/request-info
I have included a shot of what my form looks like on my iphone5s, as you can see if you look at the bottom of the shot, the layout of the page is pushed slightly to one side on the right, and the form overlaps the edge and is not contained within the frame.

Hi,
Thank you for sending your screenshot.
I think, the problem is in your container. The width of your your form is is larger than your container.
Let me try to figure out the issue. I will get back to you on this soon.

I have inspected your website on my Samsung Galaxy S4 and I do not see that the page is pushed to one side.
Can you please try to embed your form with iFrame code to see if that will resolve your issue on iPhone. http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
