I just noticed that I'm having this problem also. It's troubling and I'd like to track down the problem ASAP. Would it help to try the above forms (since this post is almost a year old)?
I know at least 2 forms (out of at least a couple dozen) have not made it to my inbox. I can tell you which ones didn't make it if that helps.
Let me know if you want me to start a new topic, although sounds like the same/similar problem as OP.

Your mail server must be using DomainKeys to detect whether an email message really came from the address specified in the message, and blocks it if otherwise.
To solve this problem, you must either turn this off or make your form conform to its rule by setting the form notification's sender name and address to JotForm and firstname.lastname@example.org respectively.
If you prefer to keep your current notification settings and would like to turn off DomainKeys, you may need to contact your hosting provider's support department for assistance.
At first I wanted to send notification directly to hotmai or yahoo mail but none arrive, so my solution was to send the notification mail to my host account and redirect from there to yahoo or hotmail.
hope this is usefull to some other jotform friends.
I don't think hostgator uses domainkeys, but I'll change the sender name and email and see if that fixes the problem. Thank you!
Im thinking about upgrading our Data Recovery Mail in Form to something more user friendly.
