Tables changing format; radio buttons not showing

In edit mode, my tables look fine. When I view in URL, preview, or edit in design mode, the radio buttons are replaced with the column labels. 

The form was converted from Adobe Forms Central.

I have tried deleting the row, saving, then going back in to add and the problem returns. 

I ended up deleting the entire table and reinserting to fix the issue. Still not sure why it occurred the first time.
