Our form no longer shows on our website. When trying to have a preview of the form, I receive message saying that the form has been disable. This must be a mistake since we only have one form running and it is a contact us form.
Can you please help us quickiy with this ?

Our apologies. You have been auto suspended for some reason. I think this is because of the blank forms in your account. You may consider purging them in your trash folder. I have just reactivated your account. The form should be up now.
