How do I re-enable my Form?

    To whom it may concern,

     

    I was running a test of my form to make sure this is something my organization can use. I got through 4 test applications and my account was disabled. 

     

    I deleted those test applications and would like to re-enable my form. Can you reactivate our coding so this form works on our webpage?
