Why have you disabled my form?

    I've checked your form and a lot of your forms are asking for sensitive information like credit card details and SSN. Here's an example of it:


    Please read our "Terms of use" page, especially the "Phishing" section. The forms violated our terms which resulted to your account being suspended. We can reactivate your account, but we will need to delete the fields that has violations on it, you will then need to agree that you will not create similar forms, it will only lead to your account being suspended.

    If you are collecting payments, it is a must to use the payment integrations that we have. You need to setup an order form in this case, here's a guide that you can follow: http://www.jotform.com/help/165-Setting-Up-Your-First-Order-Form 

    You can also set the payment integration to "Authorization Only" if you do not wish to automatically charge your customers. Here's a guide: http://www.jotform.com/help/292-How-to-Enable-Payment-Authorization
