Form and submissions are missing

    I created a form in March of 2015 with a Google sheet integration. I went to use the form today and the form and submissions are missing from jotform.com. I have the Google sheet detailing the 26 submissions from March 2015 to October 2015. Is there anyway to recover the missing form and ensure that it is integrated with the Google sheet. The form was titled Vehicle Damage and Repair Report. The form number is 50604289701150. The url is http://www.jotform.us/form/50604289701150.
