the form has lost some of its drop down boxes making it un usable. this is also the case on a copy of the form that was made a few weeks ago.

I see that you are using User contributed dropdown widget on your JotForm. It seems that there is an issue with the widget and not displaying the dropdown. Let me forward the thread to our backend team to take a look at the issue. Once we have any update on the issue, you'll be posted through this thread.
Meanwhile, you may consider hiding the widget on your form and add a dropdown from Form tools so that the captured data can be stored on a different field. Once the issue is resolved with the widget, you may add these values to the widget.

It's been resolved - the User Contributed Dropdown widget should work as expected.
