I have our form setup to receive a notification email but I have yet to receive one...never even received the test one.

I took a look at your form http://www.jotform.us/form/53266007207146 Veronica and it seems to be correctly set up.
Looking into the email logs, I can see the emails being sent as well and there is no record of the emails being bounced back.
Now what this tells me is that something on your side is blocking the emails from being received in your inbox, so please do check your spam, trash and bulk folders in both your webmail account and your email clients.
Once you do you should mark all of the emails that you find as not spam/trash and whitelist our emails so that you receive future emails without any issue.
