Why won't my form embed on wordpress site?

I've followed the instructions on how to embed the form on my wordpress site, but when I do it, it comes up with the error "**form URL** Failed to embed". Then on the page itself, it just shows up with the url.

Where are you attempting to embed your form on your wordpress site?

Also, are you using wp.com or wp.org?
