Please note that your account was suspended by our system because of the high possibility of phishing. Looking into what happened I see that you had imported the Google Drive form, which is not something that we allow per our terms of use:

Phishing You agree that Company may terminate your JotForm Service immediately if a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately

I did enable your account at this time, but if you continue breaking our terms of use we do have the right to terminate the account and delete all of the forms and their data.

You are however welcomed to use the forms as long as they are not used to damage anyone in any way and the best way to make sure not to do so is to follow our terms of use.

Can you tell us if this issue is happening on the iPod when you visit the form directly: https://form.jotform.me/53274354190454 or when you visit the page on your website where the form is embedded (over iPod)?

Based on what you have said, I presume that the form is showing white since it is embedded on your website, but if you are experiencing this when you visit the form directly, could you please grab the screenshot and tell us what is the version of the iPod and its system (and browser) so that we can try to recreate the same scenario.

As far as I know the form can not be opened within the app itself, it will instead open up in the Safari or Chrome browser (the default one) in which it should be shown.

I will however ask my colleagues that work on the app to get in touch with you if the new version shows the form within the app itself as well.

I would just like to confirm that the form is only not showing for you within the app, but it otherwise works properly - when opened in some browser on your iPod?

Yes its opening if I'm sliding the form name and then i can view form same as fill it in the browser. But its taking time while he is downloading the form. I was expected that we can fill the forms in App itself. Please let me know if we can able to do this in the future.

    Thanks for your help

I see. This is not currently possible with the app, but I have mentioned this to the developer of the app and it is added to the list of possible features.

Seems that once on that list, it is taken into consideration for new version, but I must say that it might and might not be implemented. We do however announce new versions on our twitter channel @jotformsupport so feel free to follow us there to hear once new versions our out and of course if viewing forms within the app is one of the latest features. :)
